This site does not include a health questionnaire, account, comment form or symptom journal. The question sheet offers fixed prompts; it has no fields for health details. You decide whether to add private notes outside this website.
Controls stay in the open page
Reading size, library filtering and prompt choices use temporary browser state. These controls do not send their values to a server or write them to local or session storage. Reloading resets the choices.
Downloads and printing
The text download is created in your browser and contains only your chosen fixed prompts plus blank space for notes. Printing uses the browser’s print dialog. Your device and any printing service you choose control the saved or printed copy.
Hosting and external pages
Hosting and network services process ordinary request information needed to serve pages. We have not added advertising pixels or an analytics script to this publication. External sources and provider pages have their own privacy practices.
Commercial referral parameters
